| Admission Process |

Lincoln Park Hospital wants to make your stay as comfortable as possible. Your physician has arranged for your admission. Please bring with you any information or paperwork given to you by your physician. Copies of Advance Directives or Powers of Attorney for Healthcare you provide will be placed in your medical record. All information obtained during your hospitalization will be held in strict confidence.
Pajamas, nightgowns, robes, slippers, toiletries, a photo identification, your insurance card(s) and a copy of your advance directive (Living Will/Directive to Physician and Durable Power of Attorney for Healthcare) are the only personal items you will need to bring with you to the hospital. If your insurance company requires submission of claim forms, please bring those with you also.
Please leave all medication at home unless your physician has instructed you otherwise. However, bring a list of medications with dosages that you are currently taking.

| Registration |
| Upon arrival to the hospital, patients should report to the Admitting Registration Department. The Admitting Registration department is located on the first floor, right of the Main Reception Desk as you enter the hospital. You should bring a valid driver's license or a State ID and any orders or referrals provided to you by your physician. You will be asked to sign various forms related to your admission. |

| Valuables |
| While care is taken to protect your personal belongings, we ask that you avoid bringing valuables (large sums of money, jewelry, credit cards, etc.) to the hospital. We encourage these items be either left at home, sent home with family or placed in the hospital safe. A hospital safe is available at the time of admission for securing the valuables you must bring with you. Eyeglasses, contact lenses and dentures should be kept in a protective container when not in use. The hospital is not responsible for valuable items retained by or in the patient’s possession. |

| Release of Information |
| Information about your health care is confidential. Lincoln Park Hospital recognizes the importance of protecting your privacy and has developed numerous mechanisms to maintain confidentiality. Except as specifically authorized by state law, you have the right to determine the information that the hospital may release about you. Unless you request otherwise, your name is added to the hospital’s patient roster upon your admission. This allows you to receive telephone calls, flowers, mail and visitors. Your religious preference may be provided to ministers upon your request. The hospital may acknowledge your presence and provide a one-word condition report, upon request.
You have the right to request that no information be released, except that authorized by law. If you choose to be a “no information” patient, your presence will not be acknowledged, and you will not receive telephone calls, flowers, visitors, etc.
If the patient is a minor, the parent or legal guardian has the authority to make these decisions.

| Patient ID Wristband |
| Please do not remove your patient identification wristband. To ensure that all medications, treatments, and tests are administered to the correct patient, hospital staff is instructed to double check that the name on the patient order matches the name on the patient’s wristband. |

| Foreign Language Interpretation |
| It is the policy of Lincoln Park Hospital to ensure adequate communication between patients and hospital staff when language barriers exist. Please contact your hospital caregiver when language interpretation is needed (including sign language). Your caregiver will make the appropriate arrangements for an interpreter. |

| Services For The Disabled |
| Lincoln Park Hospital is accessible to disabled patients. A Telecommunication Device for the Deaf (TDD) is available for your use. Translation services and aids are also available and, if necessary, the hospital can access community resources to become advocates for disabled patients. It is the patient or family member’s responsibility to contact the Nurse’s station in a timely manner to make appropriate arrangements. |
