| Methods of Payment |
| A registration representative will verify your insurance coverage and obtain benefits prior to or on the day of service. The registrar will be able to discuss with you the benefits payable by your insurance plan and your financial obligation to the hospital will be. Please contact a Lincoln Park Hospital financial counselor at 773-883-2731 if you have any questions concerning your benefits or payment arrangements. Charges are estimated until you are discharged and all charges have been posted to your account. Acceptable methods of payment are Visa, MasterCard, Discover, cash or a personal check. Payment in full is expected or financial arrangements must be made prior to discharge. If payment is not received within 45 days of the billing date, Lincoln Park Hospital will contact you directly for full payment.

| Co-payments and denial of insurance benefits |
Most insurance plans do not normally provide full coverage of your hospital bill so financial counselors are available to assist. Any portion of your bill not paid by your insurance carrier becomes your responsibility. Denial of insurance benefits will result in the balance becoming your responsibility. Please contact your carrier directly regarding any delays or denial of payments. | Self-pay |
If you do not have insurance, payment in full is expected prior to discharge. If you are unable to pay, financial arrangements must be made. A financial counselor will review your account to determine if you qualify for any type of governmental assistance or charity care. |

| Physician Billing |
Your hospital bill is separate from your physician's bill. Additional billings for professional charges may also come from your personal physician, anesthesiologist, emergency room physician, radiologist, pathologist, or other specialists involved in your treatment. You need to contact these services directly since the hospital does not handle these charges. |

| Financial Counselors |
Our Lincoln Park Hospital financial counselors are available to assist you in resolving the financial aspect of your hospital stay, or outpatient service. Prior to service, discharge, or during your stay, our counselor may meet with you to discuss balances not covered by insurance, self-pay balances, and obtain additional financial/demographic information that might be needed to resolve your account. Financial Counselors are located on the first floor, in the Admitting Office and can be reached at 773-883-2731. |
